Add: attribution example

### What/Why this PR?

* To make it easy for developers correctly attribute. A simple example template. Otherwise,  as in my case, I had to look at Creative Commons Attribution example, and cobbled together all the information etc.
* Hopefully this is useful. If not, please feel free emend/edit/close.

My hearty thanks to contributors and to all who make this project possible.
